Consultation: Your First Visit
Your initial appointment will begin with a review of your health and dental history, followed by an oral examination with Dr. La Puma. Often this includes the review of current, diagnostic radiographs that you may provide or we take digitally in our office. Dr. La Puma will explain your diagnosis and treatment options. On rare occasion, surgery can be performed the same day as the consultation; however, most treatment will require a second appointment to provide treatment on another day.
Please assist us by providing the following information at your first appointment:

Your surgical referral slip and all provided x-rays

A list of all medications you are presently taking

If you have medical or dental insurance all necessary
forms and insurance card
All patients under the age of 18 must be accompanied by a legal parent or guardian at the consultation visit.
Financial Information
For your convenience, we accept Visa and MasterCard, cash, and check. Payment plans are offered prior to surgery through Care Credit at 6 months/no interest under $1,000 and 12 months/no interest for balances over $1,000. We deliver the finest care at the most reasonable cost to our patients. Our fees are usual and customary for our area. Payment is due at the time service is rendered. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage. You will receive a monthly statement for any open balance and your prompt remittance is appreciated. Please understand that payment of your bill is considered part of your treatment.

We work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. Services in our office almost always fall under dental, not medical coverage.
We are in network for Delta Dental and out of network for all other insurance companies. As a courtesy, we will bill treatment to other insurance companies so that you may be reimbursed according to your plan provisions. We will also preauthorize treatment on request to other dental insurances. All balances are your responsibility whether your insurance pays or not. We are not Medicare or CenCal providers.
